Skilled Home Support is a nurse-owned clinical services company based in Oakdale, Minnesota, founded and operated by a licensed nurse with eight years of clinical experience, including specialized post-transplant care. Upon licensure, the agency will deliver skilled nursing services in the home across seven Minnesota counties: Hennepin, Ramsey, Washington, Goodhue, Scott, Brown, and Blue Earth. Services will include skilled nursing assessment and care planning, medication management, diabetes and blood pressure management, post-hospital transitional care, wound care, catheter and central line care, in-home point-of-care testing, and coordination with families and health care providers.
